Admissions Process

Before moving in, a member of our nursing team will carry out a comprehensive assessment to determine individual care needs and ensure Bryn Awelon is the right fit. We offer:

  • Planned admissions
  • Emergency or Intermediate Care admissions (subject to availability)
  • Respite care placements, based on care needs and bed availability

Upon admission, we develop a Personalised Care Plan in collaboration with the resident, their family or advocates, and medical professionals. This includes:

  • Risk assessments
  • Daily care needs evaluation
  • Regular reviews by the nursing team
  • A Multi-Disciplinary Team (MDT) meeting within a few weeks to assess progress

Care plans are reviewed monthly and updated as needed. Immediate adjustments are made if a resident’s condition changes.

We are transparent about fees and any top-up charges, which will be clearly discussed during the admissions process. Families are encouraged to visit the home, ask questions, and take home an informational brochure to learn more.
